Started getting a CAPTCHA when getting the access token? Frequency and how to remove if possible?

I received a CAPTCHA a request to fill out CAPTCHA when getting the access token.
When TOTP is enabled on the account why would a CAPTCHA come into play?
What is the frequency of this occuring? Any workaround?

CAPTCHA is only added to login auth when you have entered the wrong user credential more than 3 times. You can re-enter the correct credential and CAPTCHA will be removed.
